Welcome Raiders! I am your school nurse, Mrs. Gold. I am available to help students and staff who require medical evaluation and care during the day. I also ensure that our students have complete immunization records, and provide prescribed healthcare treatments.
If students need to take any medications at school, these Frequently Used Health forms MUST be on file and up to date:
- Parents who would like their students to have over-the-counter medicines like (Tylenol and cough drops) for minor ailments during the school day need to complete Discretionary Medication Form.
- Students who require prescribed medications for medical diagnoses related to asthma, diabetes, seizure, anaphylaxis, or other medical issues will need to have the doctor complete the Request to Administer Medication Form


If your child has not received the necessary immunizations by
September 18, he or she will be excluded from school!
Walk-ins for school immunizations will be accepted at all seven health centers from 9 AM to 3 PM from August 30 until September 30. See the link/pdf below for locations and times.
